29CFR1926.453(b)(2)(v): A harness was not worn with a lanyard attached to the boom or basket when working from an aerial lift:   (a)	On or about December 9, 2020, for the employee exposed to fall hazards of approximately 30 feet while installing siding on a five story apartment building who was not attached to the aerial lift anchor point with a lanyard at or near 215 South First Street in Bismarck, North Dakota.
