29 CFR  1910.126(a)(2) :Each tank was not equipped with an alarm that would sound if the temperature of the liquid came within 50 degrees F (10 degrees C) of its flashpoint (the alarm set point):  On or about January 27, 2021, employees working with the oil quench tanks were exposed to a fire hazard when the quench tanks did not have an alarm that would sound if the liquid came within 50 degrees F of the oil's flashpoint.

29 CFR  1910.146(c)(2):The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  On or about January 27, 2021, employees working in the atmospheric furnace area were exposed to fire, inhalation, and struck-by hazards, from a permit required confined space without any effective means to warn the employees of the danger posed by the permit space.
