29 CFR  1926.350(a)(1): The employer did not ensure valve protection caps were in place and secured.   (a) Jobsite located at 4601 Market St., Philadelphia, PA: The employer failed to protect employees against fire and struck-by hazards in that an acetylene gas tank, stored on a ready-to-use welding cart that had not been used for at least 4 days, did not have valve protection caps in place and was connected to a damaged Victor 350 acetylene gas regulator on the first floor, south side near stair tower 3.   (b) Jobsite located at 4601 Market St., Philadelphia, PA: The employer failed to protect employees against fire and struck-by hazards in that an oxygen gas tank, stored on a ready-to-use welding cart that had not been used for at least 4 days, did not have valve protection caps in place on the first floor, south side near stair tower 3.   (c) Jobsite located at 4601 Market St., Philadelphia, PA: The employer failed to protect employees against fire and struck-by hazards in that an acetylene gas tank, stored upright on the floor in front of a gang box for at least 3 days, did not have a valve protection cap in place on the first floor, south side near stair tower 3.   Condition noted on or about Jan. 27, 2021.

29 CFR  1926.350(a)(9): Compressed gas cylinders were not secured in an upright position at all times: Note:  This paragraph does not apply to short periods of time while cylinders are being hoisted or carried.   (a)  Jobsite located at 4601 Market St., Philadelphia, PA: Compressed gas cylinders were not secured in an upright position as required in that an acetylene gas tank, stored upright on the floor in front of a gang box for at least 4 days, was not stored in definitely assigned places away from locations where cylinders may be knocked over or damaged by passing or falling objects. The acetylene cylinder was located on the first floor, south side near stair tower 3.   Condition noted on or about Jan. 27, 2021.

Condition noted on or about the 3-month period of October 27, 2020 - Jan. 27, 2021.

Oxygen and fuel gas pressure regulators, including their related gauges, were not in proper working order while in use:   (a) Jobsite located at 4601 Market St., Philadelphia, PA: The employer failed to protect employees against fire and struck-by hazards in that a damaged Victor 350 acetylene gas regulator, which was missing its plastic lenses and contained bent pop-out backs on both gauges, was connected to an acetylene gas cylinder on a ready-to-use welding cart on the first floor, south side near stair tower 3.   Condition noted on or about the 3-month period of October 27, 2020 - Jan. 27, 2021.
