29 CFR 1926.350(a)(1): The employer did not ensure valve protection caps were in place and secured. (a) Jobsite located at 4601 Market St., Philadelphia, PA: The employer failed to protect employees against fire and struck-by hazards in that an acetylene gas tank, stored on a ready-to-use welding cart that had not been used for at least 4 days, did not have valve protection caps in place and was connected to a damaged Victor 350 acetylene gas regulator on the first floor, south side near stair tower 3. (b) Jobsite located at 4601 Market St., Philadelphia, PA: The employer failed to protect employees against fire and struck-by hazards in that an oxygen gas tank, stored on a ready-to-use welding cart that had not been used for at least 4 days, did not have valve protection caps in place on the first floor, south side near stair tower 3. (c) Jobsite located at 4601 Market St., Philadelphia, PA: The employer failed to protect employees against fire and struck-by hazards in that an acetylene gas tank, stored upright on the floor in front of a gang box for at least 3 days, did not have a valve protection cap in place on the first floor, south side near stair tower 3.
