29 CFR  1928.57(a)(6)(iii): Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment.  a) Maintenance Shop:  On or about January 27, 2021 and at times prior to, Coma Farms had not provided sufficient equipment safety training to its employees, including the replacement of the conveyor belt for the Logan Potato Bed.

29 CFR  1928.57(a)(11): Whenever a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish the following in 1928.57(a)(11)(i-iii):  a) Maintenance Shop:  On or about January 27, 2021 and at times prior to, employees do not disconnect the power and/or disengage PTO power before performing maintenance activities such as replacing the conveyor belts on the bulk beds on trucks that are designed to transport farm produce.
