At the jobsite, employees operating Pasolode nail guns while performing framing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
