29 CFR  1926.453(b)(2)(v):A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) Headwaters Academy, 1005 Durston Road, Bozeman, MT: On or about January 28, 2021 and at times prior, employee(s) were exposed to a catapult and subsequent fall hazards of approximately 30 feet to the ground below while operating a Genie S-45 aerial lift without fall protection.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.  a) Headwaters Academy, 1005 Durston Road, Bozeman, MT: On or about January 28, 2021 and at times prior, employees were exposed to fall hazards of approximately 12.5 feet to the ground below on east side of building and approx. 30 feet to the ground below from a vertical wall on west side of building, while performing roofing activities, without fall protection.
