29 CFR  1910.28(b)(3)(ii): 29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems.   A) In front of  ammoniator, on or about February 2, 2021 and or times prior to,  a steel grate covering  a floor chute had openings approximately 4 inches by 6 inches thereby exposing employees to a trip hazard.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:   A)  By Mixer hopper, on or about January 25, 2021, and or times prior to, the tail pulley drum on the feed belt conveyor was not guarded, thereby exposing employees to caught-in -hazards and rotating tail pulley drum hazard.
