29 CFR  1910.177(f)(5): Tires on multi-piece rim wheels outside of a restraining device were inflated in excess of the pressure required to force the tire bead onto the rim ledge and create an airtight seal with the tire and bead:  Location: Garage  An employee standing within the trajectory zone exceeded the pressure to set the bead while inflating a tire mounted on a split rim, causing the tire to explode and injuring the employee.
