29 CFR  1926.25(a):  During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  South side of building.  On or about February 3, 2021, employees were exposed to physical hazards, while removing wood and materials from the edge of a roof, with a pile of debris immediately below.

29 CFR  1926.501(b)(10):  Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted:  South side of building.  On or about February 4, 2021, employees were exposed to a fall hazard of greater than 6 feet, while removing wood and materials off the building, without using fall protection.  JPD Environmental and Demolition, Inc was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 1472099, citation number 1, item number 2-a, and was affirmed as a final order on June 12, 2020, with respect to a workplace located at 130 Valiant St., Bldg. 222, Goodfellow AFB, TX  76908.

On or about February 4, 2021, employees were exposed to a fall hazard of greater than 6 feet, while removing wood and materials off the building, without having received effective training in the hazards of a fall.  JPD Environmental and Demolition, Inc was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 1472099, citation number 1, item number 5, and was affirmed as a final order on June 12, 2020, with respect to a workplace located at 130 Valiant St., Bldg. 222, Goodfellow AFB, TX  76908.
