29 CFR  1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with the procedures required by paragraph (c)(4) of this section including, but not necessarily limited to, the following specific requirements:  a)  On or about May 5, 2021, a group lockout device was not affixed to the jumbo printer slotter machine to ensure each individual performing work on the equipment was protected from the unexpected release of stored energy.

29 CFR  1910.147(f)(3)(ii)(D):  Each authorized employee did not affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and did not remove those devices when he or she stops working on the machine or equipment being serviced or maintained:  a)  On or about May 5, 2021, employees who were performing maintenance on the jumbo printer slotter machine did not affix individual locks and/or tags to ensure equipment was not re-energized prior to work being completed.
