Warning signs and plaques were not deployed in the temporary traffic control zone to inform road users that work was taking place or what actions the road user must take to drive through the temporary traffic control zone, thereby exposing employees in the temporary traffic control zone to struck-by hazards.  The lacking signs included, but were not limited to, advance warning signs and a reverse curve sign.  B.) Jim Telliho dba Telliho Tree Service, 124 South Main Street, Springboro, PA  16435, on or about February 4, 2021 and times prior thereto:  An employees was performing flagging duties as part of the tree service operations.  Advance warning signs and plaques were not deployed ahead of the flagger station in the advance warning area to inform road users that work was taking place or what actions the road user must take to drive through the temporary traffic control zone, thereby exposing employees in the temporary traffic control zone to struck-by hazards.    C.)  Jim Telliho dba Telliho Tree Service, 124 South Main Street, Springboro, PA  16435, on or about February 4, 2021 and times prior thereto:  An employees was performing tree service operations with an International 4300 SBA 4x2 truck (VIN:  1HTMMAAP14H674417) equipped with a Versalift aerial lift (M/N:  VO255, S/N:  CV030032) positioned in the northbound lane of the roadway.  The channeling devices (i.e. cones) used for the tapper of the transition area were dirty, discolored, worn and faded, thereby exposing employees in the temporary traffic control zone to struck-by hazards.  D.) Jim Telliho dba Telliho Tree Service, 124 South Main Street, Springboro, PA  16435, on or about February 4, 2021 and times prior thereto:  An employees was performing tree service operations with an International 4300 SBA 4x2 truck (VIN:  1HTMMAAP14H674417) equipped with a Versalift aerial lift (M/N:  VO255, S/N:  CV030032) positioned in the northbound lane of the roadway.
