29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a)  Wayne's Construction Service, at 1090 King Street, Denver, CO:  On 2/4/2021 the employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to excavations to control or eliminate any hazards or other exposure to illness or injury.  Two employees entered an approximate three to five foot deep, vertically walled excavation to widen it.  One of the employees had not received effective training regarding excavation hazards.  This condition exposed the employee to a cave-in hazard.
