29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) At the Jobsite, on or about 02/05/2021 and at times prior therto, in the excavation approximately 12 feet long, 9 feet wide and 6 feet 9 inches deep on the north end and 8 feet deep on the west end, employees working in the excavation were not provided with a ladder every 25 feet, exposing the employees to the hazard of cave-ins. b) At the Jobsite, on or about 02/05/2021 and at times prior thereto, in the excavation approximately 7 feet long, 10 feet wide and 6 feet deep, employees working in the excavation were not provided with a ladder every 25 feet, exposing the employees to the hazard of cave-ins.

29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation. s) At the jobsite, on or about 02/05/2021 and at times prior therto, in the excavation approximately 30 feet long and 2 to 7 feet wide and up to 11 feet deep, an employee entered the excavation to check for a sewer line leaking while water was accumulated in the bottom, exposing the employee to the hazard of cave-ins. b) At the jobsite, on or about 02/05/2021 and at times prior thereto, in the excavation approximately 12 feet long, 9 feet wide and 6 feet 9 inches deep on the north end and 8 feet deep on the west end, employees were working to install water lines while water was accumulated in the bottom, exposing the employees to the hazard of cave-ins.
