29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about 02/08/2021, and at times prior, at 2700 and 2100 Wedgewood Plaza Circle at the Abbey in Riviera Beach, Florida, 33404, employees working on the roof of 2-story structures did not have any fall protection exposing them to a fall hazard of up to approximately 25-feet to the ground.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:)   On or about 02/08/2021, at 2100 Wedgewood Plaza Circle, in the Abbey, Riviera Beach, Florida,  employees were using an aluminum extension ladder to access the roof of a two story  building.  The ladder did not extend at least 3-feet above the landing exposing employees to a fall hazard.

29 CFR  1926.1053(b)(16):Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked "Do Not Use" or removed from service:  On or about 02/08/2021, at 2100 Wedgewood Circle Plaza, in the Abbey at Northake, Riviera Beach, Florida, employees were exposed to a fall hazard while using an aluminum extension ladder with bent rungs.
