29 CFR  1910.134(e)(1):  The employer did not provide a medical evaluation to determine the employee's ability to use a respirator before the employee was fit tested or required to use the respirator in the workplace:  (a) In the glazing department,  the employer did not provide a medical evaluation to employees who were provided and required to use  N95 filtering facepiece respirators while performing painting activities.

29 CFR  1910.134(f)(2):  The employer did not ensure that a employees using a tight-fitting facepiece respirator were fit tested prior to initial use of the respirator:   (a) In the glazing department,  employees who were provided and required to use tight fitting respirators, including but not limited to N95 filtering facepiece respirators while performing painting activities had not received an annual fit test.
