On the second floor of the building:  On or about 2/1/2021, employees were not protected from falling while working in close proximity to uncovered floor holes measuring approximately 12-inches by 14-inches in size.

On the second floor of the building On or about 2/1/2021, the employer covered a floor hole approximately 4-feet by 8-feet in size with a piece of plywood, and did not mark the plywood with the word Hole or Cover.
