A safety and health program with policy, hazard assessment, and site work rules was not developed and/or implemented at this job site.       a) Jobsite located at 100 S 6th St, Philadelphia, PA: The employer had not initiated, developed or implemented a safety and health program to include, but not limited to, identification and control of hazards, training and education, and work safety rules and procedures to enable employees to recognize and avoid unsafe conditions and hazards.  Condition noted on or about February 10, 2021.

29 CFR  1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   (a) Jobsite located 1413-1441 N American St, Philadelphia, PA: An employee was working from the upper level platform of a tubular welded frame scaffold that did not have guardrails installed on all open sides, while not utilizing a fall protection system, thereby exposing the employee to 28-foot fall hazards.  Condition was observed on February 10, 2021.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold, or work platform, trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:     (a) Jobsite located 1413-1441 N American St, Philadelphia, PA: The employer did not provide an effective training program for employees performing masonry work from a tubular welded frame scaffold, thereby exposing employees to 20- and 28- foot fall hazards.   Condition was observed on or about February 10, 2021.
