29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) There was no frequent and regular inspection program being maintained in use by a competent person to ensure the jobsite, materials and equipment such as protection of energized construction temporary string lights lamp/bulbs from physical accidental contact or breakage damage, energized 400 volt breaker panel interior uninsulated energized lugs and wiring being protected by guarding arranged and of such strength as to prevent physical damage and adequately protected elevator doorway wall openings were being maintained and utilized in a safe condition.

29 CFR 1926.403(i)(2)(ii):In locations where electric equipment operating at 600 volts, nominal, or less is exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: Located in the central area of the basement adjacent to the elevator shaft there was an observed energized 400 volt breaker panel cabinet without the face or trim cover to effectively close the breaker panel exposing employees to the lack of adequately guarding live parts, in that the face panel opening and interior uninsulated energized lugs and wiring were covered or partially with cardboard which was not so arranged and of such strength as to prevent physical damage.

Located along various floor / stairwell areas from the basement to the fifth floors of the residential structure employees were observed working and accessing adjacent to inadequately protected elevator doorway wall openings in that there was unsecured plywood or sheet metal laying unsecured in front of the elevator doorway wall openings exposing employees to fall potentials up to in excess of 40 feet.
