This exposed employees to struck by/fall hazards.

At a commercial construction site located at 222 W 75th St, KC MO -- Employees were unloading material from the 4th floor window and were not utilizing any kind of personal fall arrest system. This was documented on or about 02/10/2021.

29 CFR  1926.602(c)(1)(viii)(A):The safety platform was not firmly secured to the lifting carriage and/or forks.  At a commercial construction site located at 222 W 75th St, KC MO -- a platform was not properly secured to the forks of the off-terrain fork truck when raising materials up to the 4th floor window. This exposed employees to struck-by hazards, and fall hazards as the employees removed material from the platform.
