29 CFR  1926.502(d)(15):  Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:   Jobsite located at 63 Elm Street/158 Forest Street, Manchester, Connecticut:  On, or about, February 11, 2021, ropes and slings used an anchor points for personal fall arrest equipment were not suitable for use in that:  A.	A Lift-All Tuflex 10 foot long sling was used in a choker hitch to provide an anchor point utilizing steel anchors mounted on the top of the chimney structure.  The identification tag for the sling stated that the maximum capacity of the sling in a choker configuration was 4,200 pounds.    B.	Several pieces of rope were looped around steel anchors on the top of the structure to allow employees to tie-off while working on the top of the structure and if needed while stepping on/off the structure.  The pieces of rope were could not be identified and did not appear to be rated for use as anchor components for personal fall arrest systems.
