29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about February 16, 2021, at the Odessa Jobsite, employees performing roof work were exposed to a 10 foot 9 inch fall hazard, in that, fall protection was not used on the 5:12 pitched roof.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about February 16, 2021, at the Odessa Jobsite, employees performing roof work were exposed to a 10 foot 9 inch fall hazard, in that, the ladder side rails of the Werner ladder did not extend 3 feet above the upper landing surface.
