29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  A. Thermocolors LLC dba Rhetech Colors  2901 West Monroe St., Sandusky, Ohio 44870: On or about February 13, 2021, an employee was exposed to amputation injury in that the B62 Strand Pelletizer and cutting rotor was not locked out and energy dissipated prior to the performance of unjamming activities. The employer did not ensure energy control applications steps to control the electrical energy and kinetic energy sources were implemented prior to employees accessing the pinch point to remove materials from the cutting chamber. The employer did not:   - Identify all types and magnitudes of energy sources.  - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.  - Verify that isolation and de-energization of the machine or equipment had been accomplished.

Employees tasked with cleaning and maintenance activities of the B62 Strand Pelletizer and cutting chamber were exposed to amputation and caught in hazard in that the equipment was not adequately de-energized.
