29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) At 2046-2050 Sea Cliff North Dr. Daphne, AL (Ground Area): On or about February 12, 2020, and at times prior; the employer did not ensure that hard hats were used by employee(s) on the ground while worker(s) approximately twenty-one feet (21') above were performing roofing activities; exposing employee(s) to struck-by hazards from overhead falling/flying materials and equipment.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At 2046-2050 Sea Cliff North Dr. Daphne, AL (Roof Area): On or about February 19, 2021; the employer required his employees to perform roofing activities without fall protection; exposing employees to fall hazards of up to approximately twenty-one feet (21') from the eave to the concrete and ground below.
