29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger operating cycles:   (a)Alvord-Polk, Inc. at 205 Industrial Boulevard, Lake Preston, SD 57249:  On or about and at times prior to February 19, 2021, the employer did not ensure that employees were protected from the point of operation involving the Milltronics VM15 Vertical Milling Machine. The machine was originally equipped with magnetic door interlocks that had been removed which allowed the operator to open the door while the machine is in operation exposing the employee to the point of operation. This condition exposed employees to lacerations associated with contacting the rotating tool.
