29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:: a- Stair Tower, 129 Hartford Road, Medford NJ: On or about 02/23/21, a company worker shoveling cement from a pan and performing other related activities was exposed to fall hazards in excess of 17 feet the ground surface below when working from a wooden pallet that was that was resting, unsecured, on the forks of a JLG 10054 T4F all-terrain fork truck. This piece of equipment was not specifically designed by the manufacturer to support a scaffold platform.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section: a- Stair Tower, 129 Hartford Road, Medford NJ: On or about 02/23/21, a company worker shoveling cement from a pan and performing other related activities was exposed to fall hazards in excess of 17 feet the ground surface below when working from a wooden pallet that was that was resting, unsecured, on the forks of a JLG 10054 T4F all-terrain fork truck. This worker was not utilizing any fall protection at the time of their exposure.
