29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, were not protected by protective helmets:  a) Jobsite: On or about February 3, 2021, and at times prior thereto, an employee was exposed to head injuries when working directly beneath an aerial lift while employees worked overhead utilizing small hand tools.

29 CFR  1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a) Jobsite:  On or about February 3, 2021 and at times prior thereto, employees performed siding activities which required the use of a nail gun and skill saws without eye protection.

29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) Jobsite: On or about February 3, 2021, and at times prior thereto, two employees, wearing fall protection but not anchoring their equipment to the aerial lift were exposed to a fall hazard from heights of approximately 10-25 feet.  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated
