29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a- 11 Grande Boulevard, Lot 6, Block 0202, Tabernacle NJ - Workers were exposed to fall hazards, in excess of 12 feet to the ground surface below, when performing siding installation and other related activities from two separate pump jack scaffolds that were missing end stops and whose mid rail/screens did not run the full length of the walking/working surfaces in use by the workers.  Condition observed on or about 02/23/21.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  a- 11 Grande Boulevard, Lot 6, Block 0202, Tabernacle NJ - Two workers were exposed to fall hazards, in excess of 16 feet to the ground surface below, when performing siding installation and other related activities from a pump jack scaffold that was not secured by a rigid triangular bracing or equivalent at the bottom and other points as necessary.  Condition observed on or about 02/23/21.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 11 Grande Boulevard, Lot 6, Block 0606, Tabernacle NJ: Two company workers were exposed to fall hazards, in excess of 11 feet to the ground surface below, when working from the pitched roof of a residential structure without the use of any fall protection. Condition observed on or about 02/23/21.  OAP Construction, Inc. was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1339719, citation number 1, item number 1 and was affirmed as a final order on 11/19/18, with respect to a workplace located at 101 - 115 Meridian Lane, Sewell NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
