29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   On the north side of the commercial building, employees engaged in roofing work on a 8:12 pitch roof without fall protection were exposed to unprotected fall hazards of approximately 18 feet to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On the north side of the commercial building, employees engaged in roofing work used an extension ladder that did not have the side rails extended three feet over the upper landing surface  exposing the employees to potential fall hazards greater than six feet.

29 CFR  1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).    On the construction site, the employer did not ensure each employee was trained to recognize and correct safety hazards associated with roofing work such as fall hazards while working over six feet, extension ladders extended three feet over the upper landing surface and personal protection equipment required on construction sites.
