29 CFR 1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Wood Shop: On or about February 24, 2021, the tongue guard over the abrasive wheel side of the Wissota grinder, model # 46, serial # 9-14, was found to have a space between it and the grinding wheel of three fourths of an inch, exposing employees to injuries from chips, sparks, and shattered wheel pieces.
