29 CFR  1910.177(c)(2): The employer did not assure that each employee demonstrated his ability to safely service rim wheels by effective performance of all tasks listed in 29 CFR 1910.177(c)(2)(i) through (viii)(a):   a)  Location: Outdoors back of building in a parking lot @ 81 Depot St. Bellingham, MA 02019  On or about 3/1/21 employee was exposed to the hazard of a split rim tire explosion when the used 10.20 large forklift tire was being installed on the machine and  the employer had not insure that the employee had demonstrated his ability to adequately perform inspection and identification of the rim wheel components and did not stand outside the trajectory zone while performing this work.
