The logs  were provided May 28, 2021.   (b) The OSHA Form 300A, Summary of Work-Related Injuries and Illnesses was requested for calendar years 2018, 2019, and 2020 on February 26, 2021, March 2, 2021, March 5, 2021, and March 15, 2021. The summaries were provided May 28, 2021.
