Employees of Alpha Fabrication and Machining, LLC were moving sheet steel using an Earth-Chain Model ELM-600 Magnet attached to a Hyster Model 50, LP Forklift via nylon strap   wrapped around a fork of the lift. The attachment did not sure that the load was stable or safely arranged.

29 CFR  1910.184(d):Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service.  On March 2, 2021, the CSHO observed a nylon sling, attached to a Earth-Chain Model ELM-600 Magnet, which was frayed to the point that the interior red stitching was visible. The sling was not removed from service and was available for use by the employees to move sheet steel.
