29 CFR 1910.253(b)(4)(iii):Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. On or about March 2, 2021, employees of Alpha Fabrication and Machining, LLC were exposed to potential fire and explosive hazards when a compressed gas cylinder of Oxygen was stored less then three feet away from several Acetylene cylinders and within five feet of liquid Propane cylinder.

29 CFR 1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). On or about March 2, 2021, employees were allowed to operate a Hyster Model 50, LP Forklift without having received training on how to safely operate, load and inspect the lift as specified in the standard.

29 CFR 1910.178(o)(1):Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. Employees of Alpha Fabrication and Machining, LLC were moving sheet steel using an Earth-Chain Model ELM-600 Magnet attached to a Hyster Model 50, LP Forklift via nylon strap wrapped around a fork of the lift. The attachment did not sure that the load was stable or safely arranged.

29 CFR 1910.184(d):Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service. On March 2, 2021, the CSHO observed a nylon sling, attached to a Earth-Chain Model ELM-600 Magnet, which was frayed to the point that the interior red stitching was visible. The sling was not removed from service and was available for use by the employees to move sheet steel.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about March 2, 2021, employees operating a Bolton Tools 1440G Lathe, Serial #121118, where exposed to entanglement hazards when the guard covering the chuck was not installed in the lathe.
