(a) Eastside Exteriors, LLC, on or about March 3rd, 2021, at the location of 5329 N. Abbe Rd., Elyria, OH, 44035: The employer did not communicate a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

Abbe Rd., Elyria, OH, 44035: The employer did not ensure that he employee performing roofing work on the commercial structure while exposed to fall hazards of approximately 17 feet to the ground below had received training in order to identify and mitigate fall hazards.

(a)	Eastside Exteriors, LLC, on or about March 3rd, 2021, at the location of 5329 N. Abbe Rd., Elyria, OH, 44035: The employer was using a safety monitor system as fall protection while performing roofing work on a low slope roof.  The safety monitor was on a separate walking/working surface from the employee working, exposing the employee to fall hazards of approximately 17 feet without fall protection.
