29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about February 23, 2021, mechanical punch presses were not equipped with machine guarding to prevent employee exposure to contact with the moving shuttle system bed plate during the exit stroke of the shuttle system.

29 CFR  1904.41(a)(1): If your establishment had 250 or more employees at any time during the previous calendar year, and this part requires your establishment to keep records, then you must electronically submit information from the three recordkeeping forms that you keep under this part (OSHA Form 300A Summary of Work-Related Injuries and Illnesses, OSHA Form 300 Log of Work-Related Injuries and Illnesses, and OSHA Form 301 Injury and Illness Incident Report) to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.  On or about March 2, 2021, the employer did not electronically submit their information from their 2020 OSHA 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee.
