This was most recently documented on or about January 11, 2021, when employees worked at 964 Highway AA in Alton, MO.  Employees operated a Ford New Holland (Model: L-783 / Serial No. 778577) skid steer that presented mechanical  and structural deficiencies to include leaking hydraulic fluid and missing cab guards.  Employees operated the equipment in this condition on a daily basis for approximately four hours per day.

This was most recently documented on or about January 11, 2021, when employees worked at 964 Highway AA in Alton, MO.  Employees operated a Ford New Holland (Model: L-783 / Serial No. 778577) skid steer that was not equipped with cab side screens that would prevent the operator from coming into contact with the skid steer lift arm.

This was most recently documented on or about January 11, 2021, when employees worked at 964 Highway AA in Alton, MO.  Three employees did not comply with the operator's manual for the Ford New Holland Model L-783 skid steer when they operated it without side screens or windows in place to protect the operator from the arms of the machine.  The operator's manual for the New Holland L-783 skid steer states that the machine should never be operated without screens or windows in place.

29 CFR  1904.39(a)(1): Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor.  a) On or about January 11, 2021, an employee suffered fatal injuries from a work-related incident.  The company did not report the death of the employee within eight hours.  The St. Louis Area OSHA Office became aware of the fatality approximately 44 days later, on February 24, 2021.
