29 CFR 1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: On or about 03/04/2021 employees at the Cheney Road framing project were exposed to approximately 20' falls while engaged in carpentry activities while standing on a supported scaffold. The scaffold platform was supported by a rough terrain forklift that was not designed for such use.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 03/04/2021 employees at the Cheney Road framing project were exposed to falls while engaged in residential construction activities without fall protection in the following locations: A) Employees were exposed to approximately 20' exterior falls while installing second story fascia boards. The employee was not provided fall protection. B) Employees were exposed to falls of approximately 12' while working proximal to unprotected second floor windows.
