29 CFR  1926.501(a)(2):The employer did not determine if the walking/working surfaces on which its employees were to work on, had the strength and structure integrity to support employees safely:  On or about March 1, 2021, employees were exposed to a fall of approximately 21 feet while performing roofing activities on a residential roof located at 5220 Center, Brunswick, Ohio 44212.  The employees were working on all areas of the roof and the employer did not ensure that the roof was inspected to make sure it had the structural integrity to support the employees.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about March 1, 2021, employees were exposed to a fall of approximately 21 feet while performing roofing activities on a residential roof located at 5220 Center, Brunswick, Ohio 44212.  The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.
