29 CFR  1926.451(b)(1):  Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports as follows:  a)  On or about March 4, 2021, at the new single-family residential construction site, employees were exposed to fall hazards of more than 10-feet above the ground level below while working from a tubular welded frame scaffold that was not fully planked.

29 CFR  1926.451(g)(4)(i):  Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold is released for use by employees other than erection/dismantling crews.  a)	On or about March 4, 2021, at the new single family residential construction site, employees were exposed to fall hazards of more than 10-feet while working from a tubular welded frame scaffold that was not equipped with a guardrail system or personal fall arrest system.

29 CFR  1926.451(e)(1):  When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces shall not be used as a means of access:  a)  On or about March 3, 2021, at the new single residential construction site, employees were exposed to fall hazards while ascending / descending a tubular welded frame scaffold without the use of a ladder.
