29 CFR  1910.184(c)(9):   Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s).  WORKSITE:  Each employee involved with moving the  three quarter (3/4) inch thick, 6-foot by 8-foot long steel road plate hoisted and moved using the Caterpillar hydraulic tracked excavator, with one (1) half inch alloy steel chain sling was not clear of the suspended load.  Consequently, while hoisting and moving the steel road plate the alloy steel sling disconnected from the road plate, fell, and struck an employee resulting in serious bodily injuries.
