29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about 3/9/2021, during sewer installation, located at 7821 Hunting Lake Drive in Concord, Ohio, the employer failed to ensure that employees working in a trench and exposed to overhead hazards were provided and utilized hard hats.

29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about 3/9/2021, during sewer installation, located at 7821 Hunting Lake Drive in Concord, Ohio, the employer failed to ensure that employees entering a trench greater than 4' in depth were provided and utilized a ladder. The trench was about 4.10' deep, 5.8' wide, and 24' long.
