29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   Worksite located at 2165 Hay Market Ct., Liberty, Missouri- An employee was exposed to a fall hazard in that the employee was observed standing on a load of siding suspended by a fork lift. The forklift was not designed by the manufacturer to lift employees in this manner.

29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   Worksite located at 2165 Hay Market Ct., Liberty, Missouri- Employee was exposed to a fall hazard in that the employee was using a make shift scaffold platform and was not protected from falling by the use of guardrail systems or personal fall arrest systems.
