The employer did not implement a written hazard communication program; on or about 3/8/2021. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals or materials in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area: a) Paraco Bay Shore Facility: Contract employees worked in close proximity to and handled cylinders of liquid propane gas and were not trained on the hazards of liquid propane gas; on or about 3/8/2021. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
