29 CFR  1910.178(a)(4):  Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  The Toyota electric forklift did not have a name plate with the capacity, operation, and maintenance instructions, plates, tags, or decals to identify the carpet roll attachment (Model:  RP2 75/20A, S/N:  522941).

29 CFR  1910.303(g)(1):  Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  In the Warehouse, the electrical panel did not have at least three feet of clearance, preventing employees from having safe and ready operation and maintenance of the panel.
