The Toyota electric forklift did not have a name plate with the capacity, operation, and maintenance instructions, plates, tags, or decals to identify the carpet roll attachment (Model: RP2 75/20A, S/N: 522941).

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.
