Located at 6245 Mistover Lane in Canal Winchester, Ohio: On or about March 12, 2021, the employer did not ensure that employees working below the roof edges where an existing roof was being removed above were protected from falling tools and debris, thereby exposing employees to a struck by hazard.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a. Located at 6245 Mistover Lane in Canal Winchester, Ohio: On or about March 12, 2021, the employer did not ensure that employees performing roofing work were protected from falling from the roof. Employees were exposed to fall hazards ten to twenty foot to the ground.

Effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system. Note: The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section.  a. Located at 6245 Mistover Lane in Canal Winchester, Ohio: On or about March 12, 2021, the employer did not ensure that employees performing roofing work were protected from falling from the roof due to the use of unapproved climbing harnesses. The equipment was not designed and approved for use as a personal fall arrest system.
