29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer is failing to protect employees from fall hazards while using ladders to access elevated work surfaces. This was most recently observed on March 12, 2021 at 18202 Camelback Ave, Omaha, Nebraska when 2 workers were using a portable ladder to ascend to the roof of a residential structure that was over 11 feet above the ground and the side rails did not extend at least 3 feet past the upper landing surface.
