29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite-specific procedures was not established and implemented for required respirator use:   Hospice Source, LLC located at 9852 Crescent Park Drive, West Chester, Ohio 45069:  a.)  On or about February 2, 2021, Hospice Source, LLC did not develop and implement a written respiratory protection program with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1)(i) - (ix).  The employer required employees to wear filtering facepiece respirators throughout their shifts while exposed to suspected or confirmed positive COVID-19 patients.

Hospice Source, LLC located at 9852 Crescent Park Drive, West Chester, OH 45069:   On or about February 2, 2021, Hospice Source, LLC did not provide a medical evaluation to determine each employee's ability to use a respirator before requiring the use.  The employer required employees to wear filtering facepiece respirators while setting up medical equipment for suspected and confirmed positive COVID-19 patients.

29 CFR  1910.134(k)(1): The employer did not provide effective training that covered the required elements in 1910.134(k)(1)(i) - (vii):  c.) Hospice Source, LLC located at 9852 Crescent Park Drive, West Chester, Ohio 45069:   On or about February 2, 2021, the employer did not provide employees with effective training on the effects of improper fit and usage, use of the respirator, and how to recognize medical signs and symptoms that may limit or prevent the effective use of the respirator.  The employer required employees to wear filtering facepiece respirators to protect against the SARS-CoV-2 virus while setting up medical equipment for suspect and confirmed COVID positive patients.
