29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: a) 19th floor set-back area and scaffold platform, North-West (3rd Avenue) side of building a) On or about 03/18/2021, an employee was exposed to a fall of approximately 12 feet when he jumped over an open floor gap between a scaffold platform and a set-back area on the 19th floor. The open floor gap was approximately 3 ft. wide.

29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee. Location: a) 19th floor set-back area and scaffold platform, North-West (3rd Avenue) side of building a) On or about 03/18/2021, An employee cleaning an area on the 19th floor was exposed to a fall hazard. The employee jumped over a 3 ft wide open floor gap between a scaffold and a set-back area to access the work area. The employee did not recognize the exposure to a fall while working by an exposed edge.
