29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a)  At 10740 Vaughn Road, Pike Road, Alabama:  On or about March 11, 2021 and at times prior; the employer exposed employees to worksite hazards in that the employer failed to instruct his employees working in a 9 foot deep trench in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to their work environment to control or eliminate any hazard(s) or other exposure to illness or injuries.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  a)  At 10740 Vaughn Road, Pike Road, Alabama:  On or about March 11, 2021 and at times prior;  employees were allowed to work inside of a trench measuring nine feet deep, nineteen feet wide, and fifteen feet in length that was dug in class "c" soil  without cave-in protection in place exposing the employee to crush and cave-in hazards.
