29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b)  A)  New Roof Installation - 9 Plaza PL, Edison, NJ 08817   Employees engaged in roof activities were not protected from falls by a fall protection system. An employee fell 30 feet and suffered serious injuries.    Exposure occurred on or about 03/10/2021  Guaraca Torres Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1),which was contained in OSHA inspection number 1081532, citation number 1, item number 1 and was affirmed as a final order on 11/15/18, with respect to a workplace located at 224 Meade St, Perth Amboy, NJ 08861  Guaraca Torres Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13),which was contained in OSHA inspection number 1304237, citation number 1, item number 1 and was affirmed as a final order on 04/06/16, with respect to a workplace located at 960-982 Lake Ave Clark, NJ 07066.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  A) New Roof Installation - 9 Plaza PL, Edison, NJ 08817   Employees engaged in the installation of new roof shingles were not trained in the recognition of fall hazards and the procedures for minimizing these hazards.  Exposure occurred on or about 03/10/2021
