29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 23, 2021, at a newly constructed single family dwelling in Conroe, TX, employees were exposed to a fall hazard greater than six feet when performing roofing work without a fall protection system.  PORFIRIO CASTELLNO LOPEZ WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION 1303641, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A  FINAL ORDER ON APRIL 25, 2018, WITH A FINAL ABATEMENT DATE OF MAY 8, 2018, WITH RESPECT TO A WORKPLACE LOCATED AT 3814 GLENFIELD SHADOW CT., KATY, TX 77493.  PORFIRIO CASTELLNO LOPEZ WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION 1197840 CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 2, 2017, WITH A FINAL ABATEMENT DATE OF DECEMBER 15, 2016, WITH RESPECT TO A WORKPLACE LOCATED AT 5507 CASA MILA DR., KATY, TX 77449.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 23, 2021, at a newly constructed single family dwelling, where employees were exposed to a fall hazard when using an extension ladder to access a roof without extending the ladders side rails at least three feet above the roof landing. 29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about March 23, 2021, at a residential construction site located in Conroe, Texas,  employees were exposed to eye injuries while operating pneumatic nail guns without wearing eye protection.
